Oral and oropharyngeal carcinomas are the sixth most common cancers in the world. Clear cell odontogenic carcinoma (CCOC) is an odontogenic tumor with aggressive nature. It is a rare entity with only few cases reported in available literature. The nature of its biological behavior seeks more attention of clinicians and researchers. An understanding of the same is helpful to differentiate it from other similar lesions. This text includes a detailed description of CCOC with its clinical, radiological and histological features. Recent literature describing the molecular concepts and application of immunohistochemistry is highlighted in obtaining an accurate diagnosis.
